My monitor does this every once in a while:

It looks kind of different every time. As you can see, it only happens in game; if I open steam or alt-tab, the issue doesn't occur. I'm not sure if it happens in anything other than tf2 since it hasn't yet (but I only have a handful of hours of other games since this issue has started to occur and it happens rarely..sometimes it happens like 5 times a day though). I haven't tried to do much other ensuring my gpu drivers are up to date.

other suggestions?

I used to have this problem with a hud I was using, I changed the hud and it seemed to fix it. Haven't encountered it since.

check your DVI cable (reseat on both ends, if that doesn't work test with a different one)

Does this occur in different games?

Does this occur in non-source games?

Does this occur during youtube videos?

Does this occur when using another PC with this monitor?

I currently have 3 hypothesis's for what's causing your issue.
1. DVI or Display port cable needs to be reconnected OR replaced. (poorly connected or bad cable)
2. Uninstall and Reinstall updated GPU drivers. (Bad/conflicting drivers)
3. Your GPU/CPU is overheating and/or dying.

I doubt this is your monitor cause this would happen during any kind of pixel transitions including video if that was the case and it wouldn't look like this.

not yet, like i said, haven't played them for more than just a few hours.

it sometimes happens so rarely (for example, it hasn't happened in 4 days, but that one day it happened like 3 times) that i don't have the time to use somebody else's pc for that long.

neither are overheating. they're both around 3 years old.

things i'll try:
-replace my dvi cable (when it happens and i try to unplug both ends and plug them back in, it still happens until i restart tf2. since the fix is to restart tf2, i don't see how it could be the cable either)
-reinstall gpu drivers

considering it doesn't happen outside of tf2/games(?), I don't see how it could be a monitor issue.
