I'm actually on it now, its called Isotetrinoin over here. Was on 30mg a day for a month and now I'm on 60mg a day for another three month.
Some backstory, I tried every single other method of clearing acne before this and none worked. This shit does work, honest to god. Its 100% worth going onto it.

You will have side effects, without a doubt. I've had a fair amount tbh such as:
Really dry skin, like so dry it peels in the morning.
My nose (inside) is always dry, and cracks up and bleeds.
My lips are always dry
Obviously, my face is always sore and red.
The edges of my finger nails are really dry and start to peel, which is quite painful.

None of these are life threatening and for what? 4 months of my life to get rid of my acne? Fuck yeah.
The first few months are the worst, initially since your skin just dries out and gets sore as fuck.
I use moisturiser like 3/4 times a day. Once in the morning and once when i sleep for definite. Before putting it on I recommend washing for face and even going as far to rub your skin (quite hard) with a wet towel to get rid of all the dry skin.
I always carry vaseline to put on my lips. Theres nothing I can do about my nails or nose so fuck it.

The extreme side effects are so uncommon I wouldn't worry too much, and they give blood tests to see if your body is coping.

Edit 2: Other treatments I tried were benzoyl peroxide, clindamycin, tetralysal and standard spot creams. none worked as good as this.
As for mood swings, they're not extreme at all and are prone to people who usually have them (why they ask you if you've had or are being treated for depression)

I had a really bad time on isotretinoin, but now a few years later I'd 100% say it was worth it. I couldn't eat sandwiches 'cause every time I tried to open my mouth to eat it my lips would crack. I couldn't really do sports for the last few months and a couple months after I stopped taking the pills because I had muscle pains in my stomach as a result of it. Some people have better experiences with it, but in general you probably will end up experiencing a few of the many side effects. With that said you'd probably be thanking yourself in a few years time. It's an investment of sorts, you have to ask yourself if 4-6 uncomfortable months is worth getting rid of most of your acne for good. For me it was, but it's definitely something to consider for anyone thinking of starting the program.

I took it about 8 years ago and it was the best decision ever. I also had a lot of acne on my arms and back etc. It cleared it right up in a couple of months I had next to now pimples. I mean this really really works. I didn't really have any side effects I was told not to drink and my skin would be more sensitive to sunlight while on it. I didn't notice NY dryness that I remember I always had lib balm any way so that helped

I didn't have particularly bad acne, but Roaccutane got rid of it completely. Six years after the treatment, I get one tiny zit like every two weeks and that's it. You must take your monthly blood test; from what I understand, they mostly monitor the effects on your liver, so I'd refrain from boozing during the treatment, like CommonPleb said.

Everyone already mentioned the dry skin, so be prepared to deal with that. My doctor didn't bother mentioning that bit to me. Be extra attentive to your body; you never know if you're one of the super unlucky to get a particularly bad side effect.

i've never had to use accutane for acne but i get really bad chapped lips due to allergies (not as bad as these side-affects seem to represent but still bad) and chapsticks always needed to be reapplied every couple hours, so I imagine it would be even worse in this case. But then i found paw paw ointment, shit's amazing. Feels like it lasts the entire day and then some before you need to reapply.

I guarantee it will feel 10x as good as any chapstick you can buy. Works for chapped lip and dry skin and it isn't unhealthy in any way that I know of.

My acne was never bad enough for that stuff, but it was moderate probably like most kids had. My twin sister had the same stuff, and she ended up going to the dermatologist a bunch and used proactiv like every 10 minutes or whatever you're supposed to do. Mine ended up clearing up a lot sooner than hers did and I never really used anything except soap in the shower etc.

Anyways I wouldn't take the risk with something like Accutane unless like others have said it's going to scar or is really painful. You'll outgrow that shit eventually and I know some high school kids can be dicks but anyone worth a shit won't actually care about acne. Try not to be too self conscious about it cuz everyone is at that age, whether it's height or awkward shaped boobs or acne. Easier said than done I know, but just my 2 cents~
