id probably recommend uninstalling all of your video/mouse drivers and reinstalling them separately to narrow down the cause, also make sure you run a 'clean' install when you put them back.

this is besides the problem, but type
-windowed -noborder -x 0000 -y 0000 into your launch options.
(insert your resolution in the "0000" space, for example, -x 1680 y-1050).

It'll make alt-tabbing more efficient than it was in fullscreen mode.

on any Windows when Aero is enabled. so that's everything Vista and newer, excluding the classic/aero basic theme in Vista and 7 (this was removed as an option in 8).

How many fps are you getting? The game will feel weird if it's well below 144. Double check you're not still using integrated graphics. Do you have the latest drivers?
