Are you running anything that would interfere with the kernel? Do you have a realtime AV installed? What kinds of overlays do you have enabled, e.g. mumble, fraps, etc? Have you tried deleting (after backing up) your config and starting from scratch with single-threaded and otherwise default graphics settings?

this was happening to me in year (2011-2012 on my OLD PC) when you turn off the pc and PC asks you that some programess still running then DO NOT PRESS FORCE CLOSE because you might get this immediately crashes, so i always keep to close programess (hl2.exe) by windows , i know it takes a time but , i do not want to risky again ( i solved this by not continuing Force close )
