I plus fragged a comment, then posted one of my own comments. A few seconds later, I forgot I previously plus fragged the comment, so I clicked again. After refreshing the page there are two +frags.

This is what happened. If you +'d something, then made a post, then looked at the thread again, chances are someone also +fragged it.

its a glitch. when you refresh the +frag will be gone
