I've noticed this too. I had it happening consistently in a lobby on Swiftwater the other night, but I HAVE had it happen in other maps too, though it's much rarer.
I launch with
-novid -nod3d9ex -window -noborderbut I don't use a graphics config, just settings I manually enter.
Here's my config.cfg if it'll help.
Also, Sigafoo had this happening to him on stream on pro_viaduct and Gullywash. He didn't crash, though.
I would really love to try and open these in SFM if someone can find the map files.
Try reseating the GPU and reinstalling the graphics drivers. If that doesn't work, it's time to RMA.

AndiSabinBenQ XL2411T does not have support for HDMI 1.4. If you want 120+hz@1080 you need DVI DL

Well, the big job I applied for today has a turnaround of about 2 weeks for the application process. I'm going out tomorrow and job hunting locally anyway. So keep me in mind and I'll keep you posted. It all depends on what kind of hours I find.

How are you streaming TF2? Are you pulling a game capture, or recording the screen?
It almost sounds like you're doing the latter.
Remove the wall protecting the ramp, it doesn't need to be there.
I don't think you should both open the house AND have the extra path from the 4th screenshot.
I think the door should still be on the first point, but what if you make it so that it's only partially open vertically, so you have to crouch through it before it fully opens? It discourages pushing through it, but gives the Engie an escape route when falling back. It also promotes stealthy Spy play.
