cl_blobbyshadows 0
and then obvious ones like r_shadows 1 and r_shadowmaxrendered not 0 etc.
edit: obviously fair warning cl_blobbyshadows 0 will let shadows bleed through walls but it also reduces fps fairly heavily.

make sure you play the fallout fixt version, which removes a bunch of bugs from the original game and adds a lot of nice improvements such as hd resolutions and scroll wheel support.
put -dxlevel 95 in your launch options then take it out after you launch the game.
iirc AMD stopped its dx8 support, so if you have an AMD card try rolling back drivers or switching to dx9.
also screenshots would help people diagnose the problem easier than words.
I think mat_phong, mat_bumpmap will affect it, as well as the -nops20b launch option.

mastercomscompquality.cfg would enable any effects that would give you an advantage, even if they decreased FPS. For example, muzzle flashes, shadows and enhanced sound spatialization/effects.
This is the config I think most people want, preferably with enough detailing so that we can look through it and disable anything we don't deem worth the fps loss.
On a side note do you know of any commands which make it harder to find cloaked spies, a while back I changed my config and I think spies blink differently which makes them harder to see.

Drink coffee / tea / decaff / hot chocolate is what I do.
Ah so that way it will keep the colour even when you run out of ammo to switch weapons, thats pretty neat :)

Why would he make a vtf crosshair green? One of their best properties is you can make them white and use the ingame commands to colour them. :(

The wait command is banned for a reason, using external software to play the game for you is cheating imo.
