hosting a dedicated server can be more resource intensive than expected, and coupled with a game like tf2 things might not work out that well
even in other games, such as don't starve together, the dedicated server is poorly optimized and actually takes a behemoth of a computer to both run the server without significant latency alone, let alone play the client version at the same time.
i'd recommend trying to run the server alone and kill all other programs. Then see how your friend's performances fare when you aren't playing the game at the same time.
if that doesn't help then i'm not sure what could be wrong beyond a network issue like not having a fast enough upload speed :(

what are the specs exactly on ur computer
also are you attempting to host and play on the same machine?
the distance is one thing, but if you dont' have the upload to support the data transfer rates then your server will lag and people's ping will spike

that's some crazy down but i'm afraid that upload is at least part of the problem, i assume the machine itself is pretty beefy.
I have something like 30 upload at home and people playing on my hosted tf2 server complain about slightly higher ping than they would normally experience with a chicago server
and the machine i use to host isn't what you would call modern

i'm pretty sure tf2 dedicated servers need hefty upload speeds so your net might not be cuttin' it
do you allocate memory in the launch command? maybe you're running out

you can also pipe twitch streams directly to the video player of your choice, which is quite nice imo

once you set the path in ur environment variables you can just type into a command prompt to start a stream
you can even open up a chat box with a small .bat script at the same time
start chrome --app=https://www.twitch.tv/popout/%1/chat?popout=
streamlink twitch.tv/%1 bestneedless to say this bypasses ads in their entirety and you only get the stream
so if you put those two lines in a "twitch.bat" for example, you could type in cmd "twitch etherr_" and it would open up his stream in ur video player and his chat simultaneously
