I think the command is for your own listen server and not for the server you are connected to. Also there is a bunch of cfgs in cfg folder for competitive and casual and there is fixed spread.

Clients and servers have their own separate convar values. Convar values as reported by the client do not necessarily match the server's unless the convar is marked as replicated (ex. sv_cheats, mp_timelimit). In this case, your client is reporting its own copy, which is set to the default value. This particular convar does not need to be replicated since weapon spreads are calculated server-side, presumably to make nospread more difficult to implement.

e: Oh wait, I didn't read the screenshot closely enough. That one is marked as replicated. Beats me, then.
