Game capture should not capture the overlay, make sure to untick the 'Capture third-party overlays (such as Steam)' is unticked in your game source settings.

Also make sure mat_queue_mode is on -1 or 2
Update drivers (Audio and GPU), update DirectX, validate game files, try running with -default and/or -autoconfig as launch options.
Might want to mention hardware and possible ingame settings (DX level, texture settings) as well.
Would it be possible to get a preset for frag movies/casting camera to keep the original valve look, but with harder limits to prevent crashes from happening? Or would that be Preset 0?

Also, if the resolution from your game in full screen differs from your native/original desktop resolution your GPU needs to switch between those before tabbing in/out of the game.
Why not just use CastingEssentials with binds which also work for any class, regardless of TF2 installation location?
Make sure after extracting the default hud files you also have an info.vdf in the root of your custom hud folder.
This is done by modifying the textures which make up the control points and the animations.
Animations are done with icon_obj_cap_blu/_red and possible _up versions for A/D maps like gorge and gravelpit.

The screenshot is from the BlackOut TV HUD (also used by Sigafoo for NA HL casts) and the original horizontal control points were created for the current TFLIVETV HUD.
Does a snd_restart fix it (temporarily)? It might be the length of the hitsound causing problems, try one of the default ones in your advanced options and see hear if it still happens

HudMatchStatus.res controls the player panels and background behind it.

You can make some hidden offscreen buttons to do the same functionality. Or you can change the labeltext of the existing buttons to be &a and &d instead.
imo a lot of questions you ask can simply be answered by looking at the files of other huds and trying to figure out how they solved the thing you're trying to achieve.

Try setting the scores > nemesis_width column width to 0 and hope it doesn't auto resize.
