I've had that problem before when I had my CRT. I would test 164 hertz with a resolution of 640x480 and it would go through but when I try to do it with TF2's launch options I get what you are describing. What I ended up doing is dropping my -freq value by 1 until TF2 accepted it. I ended up with a refresh rate of 144. I'm not a monitor expert but 144 is a common refresh rate preset and TF2 might only display if you have your refresh rate set to one of the commonly used presets. Maybe try 144?
