I had this problem with my G500 after about a year of use. But it's definitely my mouse, the wire was all cut up and the usb port was simply not responding. But then again... logitech.

Not sure about the FPS capping thing, but yes, do not leave -dxlevel 81/91/whatever in your launch options, I used to run into this very problem of black screening for a minute until I removed the dxlevel option from my launch options, and it got fixed. No black screen.
