as lias said, there will always be distinct differences like with viewmodel fov, damage numbers/damage indicators, hitsounds, etc.

pretty sure stvs show health and (though in most huds it normally looks different from spectator vs first person) there are ways to edit this and make it more aesthetically appealing

that being said it's hard to tell without logs.tf (which has a separate health packs stat and consequentially more accurate heal stats)
