I don't think you can isolate one specific difference. On average the average player will be a little better at everything. It doesn't make sense that the progression of DM suddenly stops at IM.
The most popular EU DM server has that setting and I personally still prefer the one that gives you half your hp back for killing someone, but nothing otherwise. As rewarding it is to get health back for doing 180 dmg to a soldier and getting the kill taken in front of you it's really annoying to fight a scout that suddenly gets healed to full since he damaged a soldier a while prior. Obviously personal preference, but I prefer just getting hp for kills.
They're gonna need to optimize the game more if they're gonna limit the graphics customization.
Doesn't sound like Valve will allow you to turn off your viewmodels. Wouldn't be a fan of that.
