Haven't played in years, reinstalled the game, fps capped at 30. I tried deleting cfg, using comanglias fps config, changing fps_max to various numbers; nothing changed. Netgraph shows the fps as starting above 50, then lowering to 30 every time I enter a server. Input delay for movement as well, mouse movement and WASD, gun firing is fine though. VSync is off, as well as all the other video options in the menu.
This a known issue?

Huh, slight update, turning VSync on and switching to windowed mode fixed it. But I want to play full screen.

Also, there is apparently an issue on Windows 10 with mouse input, so if you are getting input lag, try m_rawinput 1.
For keyboard input lag, you should be using in_usekeyboardsampletime 1.

Had the same issue until I used flame's fps method
I made the cfg file, then put it at the start of my autoexec, so it execs before my comanglia's cfg, personal preference settings and binds
(it'll give you some dumb commands like fov_desired 75 so make sure to go through the file in case there's something in there you don't want that wouldn't get overwritten by your other config files)
