I tend to have mouse delay if I run windowed, so I use fullscreen

Make sure you disable Windows Aero when you run windowed, it causes mouse lag.

For nvidia cards: Force disable vsync in drivers.
For ati/amd cards: Force disabling vsync does nothing. nothing. in windows 7. not sure about vista, but it's probably the same. having flash, other games, emulators, etc open can also cause input lag, if they enable vsync. if anything running causes vsync to be enabled you'll get input lag, period, unless you run fullscreen. aero enables vsync itself, which is the cause of a lot of problems.
