Happened to me a few times, random unjustified mumble bans. Luckily they usually don't last long, so its not a big deal.

30 minutes for first offence I think, but after that the ban could turn into a several hour or several day ban if you continue to rack up mumbility points. If you can prove you were in mumble or tried to join or whatever and there were issues, sometimes people post on the TF2C forums and an admin can remove the ban.

Not in my experience. They basically just told me to fuck off the few times I actually asked about it. One time I was even still in the Mumble with my team talking and they wouldn't remove the ban.

To be fair, going from giving competitive TF2 a chance to being Mumble banned for a reason out of your hands (without knowing it's only 30 minutes long) is a great situation to piss off potential players.
